• If we are not in-network with your plan, submit this to your insurance company for reimbursement under out-of-network benefits. 

• Self-pay rates are $160 for the initial visit and $135 for each follow-up visit.  

• We are in a network with Medicare, Anthem Blue Cross Blue Shield, Tricare, and Memorial Health Partners/Core Source. 

• We are in-network with BCBS Medicare Advantage Plans, but out-of-network with any other.

We are sorry for any inconvenience if we are out-of-network with your insurance company. We are a small specialty clinic that sees patients one-on-one. Many plans do not reimburse us enough to keep our doors open. Also, we are one of the few private pelvic floor clinics in the country that accept insurance. However, we are passionate about this specialty and believe pelvic health should be made more readily available.
